Adoption Program Volunteers' Checklist

The following checklist is provided for volunteers assisting with adoptions.

COMPLIANCE:

Volunteers performing precompliance inspections should use the current version of the Compliance Checklist. Please see that all sections are completed.

ADOPTION AGREEMENTS:

Two forms are required. The first is the Adoption Agreement. The second is the Maintenance and Care Agreement. Please see that all information has been filled in and is legible!
