Horse & Burro Adoption Forms
These adoption forms are used for adopting State of Nevada Virginia Range "Comstock" wild horses, prison trained horses from the Warm Springs Correctional Facility in Carson City, NV, horses & burros from the Sheldon National Wildlife Refuge and LRTC "rescue" animals.

The forms can be downloaded. They are in Adobe PDF format so that they will print correctly on most computer systems. They can be read and printed using the downloadable free "Adobe Acrobat Reader." The forms must be completely filled out and submitted in hard copy. The appropriate adoption fees should accompany the forms.

FORMS FOR ADOPTERS

DOWNLOADABLE FORMS:

The easiest way to acquire forms is by downloading the entire combination of all forms. This combination includes the Adoption Application, Maintenance and Care Agreement, and a Compliance Checklist. As an adopter you need to fill out the Adoption Application and Maintenance and Care Agreement. An approved wild horse volunteer, veterinarian or animal services officer needs to complete the Compliance Checklist. If you are adopting an animal from the internet you can fax these documents to us to protect your animal from being adopted by someone else, however we will need the original documents in order to process your adoption.
